The Identification and Documentation Collection consists of official records from the Office of the Government of Puerto Rico in the United States (OGPRUS). It includes the Application for Certificate of Identification and supporting documents that served as proof of migrants’ U.S. citizenship. These supporting documents may include affidavits, official correspondence, birth, baptismal, marriage, divorce, and death certificates and identification cards.

- parent's birth information
- affidavit of identifying witness
- husband's name & birth information (for female applicants)
- date of application
- method of citizenship
- photograph of applicant
- emergency contact name & address
- signature of the Chief of Identification Service
